WHAT MAKES safe harbor THE BEST CHOICE FOR A REFERRAL? |
Safe Harbor is the first court-administered adolescent substance abuse treatment program in Michigan to offer all phases of treatment: stabilization, assessment, treatment and transition services, as well as, meeting education requirements from one location. For the past 16 years, Charlevoix County Probate & Family Court has successfully provided adolescent substance abuse program services, and based on that experience results reveal that many teenagers abusing substances are also struggling with unresolved traumatic stress and emotional issues, which frequently and directly contributes to their substance abuse. Therefore, this newly enhanced program will address the deficit in traditional substance abuse programs that leave unresolved emotional, mental and traumatic stress issues unaddressed.

Making a referral
Making a referral is simple; below you will find two documents; the first is the "Referral packet". This is the first form that needs to be completed as thoroughly as possible. This helps us make a predetermination if your youth is an appropriate fit for our program. Please include any supporting documentation (assessments, evaluations, etc.) that would assist us in making this determination. The form can be emailed back to the coordinator at [email protected], or faxed to (231)547-7256. You will receive confirmation once it is received. This information is reviewed by the coordinator of Safe Harbor, Director of our Counseling staff and our Court Administrator. A determination is provided normally within 24 hours.
Once your youth is ordered into placement the next form; "Intake Packet" needs to be completed. This packet contains documents for your agency to sign and also numerous releases that the parent/guardians and youth need to read and sign. Please ensure that we get copies of insurance cards, birth certificate, SSN card, placement order and signed service agreement (this must be signed by your Court Administrator).
Now its time to bring the youth into the program. We do not normally provide transportation for this. We encourage the parent/guardian or probation officer to accompany the youth for the initial intake. This gives the opportunity to meet staff, tour the facility and ask any questions they might have.
